The Therapeutic Response in Adult Idiopathic Thrombocytopenic Purpura




Abstract
The object of this study is to evaluate the result of different therapy in adult ITP by
means of strict and uniform criteria of therapeutic response based on the response
criteria of DiFino and Cooperative Latin American Group on Hemostasis and
Thrombosis. An analysis on 60 cases of adult ITP which were treated at Department of
Internal Medicine, Severance Hospital from January 1975 to September 1986 are reported.
The result obtained were as follows.
1. The CCR to corticosteroids as initial trial of ITP was observed in 12.1%, and the
CCR to splenectomy 56.3%, which showed statistical significance(p<0.01). No CCR to
reinduction with corticosteroids was observed.
2. In CCR group after splenectomy, the symptom duration and the period from
symptom duration and the period from symptom onset to splenectomy were shorter than
those in non-CCR. And the maximal platelet count of CCR group was higher than that
of non-CCR.
3. The result of all therapeutic modalities showed CCR 27.6%, TCR 24.1%, PR 32.8%
and NR 15.5%.
4. The complication of corticosteroid therapy was observed in 17 cases, while it was
seen in 3 cases of splenectomy without delayed complication.
This study suggests that splenectomy was a major contribution to improvement of
therapeutic results in adult ITP.
